Can anyone suggest a way to get posts to import in the same sequence as the csv file? I’d like to be able to use the forward and back features of WordPress to scroll through what is essentially an ebook, but this importer doesn’t appear to keep things in order.

I would recommend either filling in the csv_menu_order field or the csv_post_date field to ensure your posts are sorted properly. The importing being out of order is more a function of WordPress than a function of this plugin.
