The JavaScript is loaded and the events are attached.
You could try the last resort…
Disable all plugins and choose a default theme. See if it works then. Then enable things one by one and see where it breaks.

Hi, I see you have a form in the main content and in the sidebar. This plugin is not designed to have 2 forms in one page. I would think, if you remove the widget from the sidebar, that the form in the main content will work.
