I checked on some of the websites I manage, that one of the causes is the template.
How can I correct this anomaly.

The free version depends mostly on your theme to cover the styling. The pro version has a styler built in where you can visually style your forms.
For the template problem, can you provide a link to the form where we can see it?

The Formidable styling is not loading correctly on your site.
