Including PHP page Not Widget Area

I apologize ahead of time if I missed a similar topic for the issue I am having. I have worked with php before and can understand certain parts of code when I look at it but am not so great at writing it myself.

I can see the php function call in the readme.txt and I added it to my category.php page but I want to be able to hide the category drop down and for the life of me can’t see how to target that option using the $instance arguments. Any help would be appreciated.
