Inline Form Issue

Hi,

I created an opt in form here (www.kristennolan.com). This is an inline form that I want to stay there. However, when I tested the form and went back to it. It says “content” instead of showing the form.

How can I remove this? I want the form to always be there even if people are having multiple submissions.
