Input Box Overlaps Form Field

I have added custom form fields to the book appointment page, but the text gets cut off by the input box. I cannot make the text any shorter because I need the clients to answer some questions (but I see that it was a previous user’s solution to the same problem). How can I make it so that the input box moves over and exists after the full field name?
