Insert Form Returns Error

    So the insert form works well, the record does end up on the sql database but returns the following error

    Couldn’t perform query (1400): SELECT FROM UserRecords WHERE Number=’5′ AND Name=’Richard Test’ AND Location=’Alabama’ ORDER BY DESC LIMIT 1 – Unknown column ” in ‘field list’.

    Any ideas?
