Are you sure you activated the plugin? Could you show a full screenshot of Plugins > Installed plugins where we can see the plugin activated And the menu not showing up?
Maybe try to http://refreshyourcache.com ?

I have cleared the cache (even thought it wasn’t appearing on other computers or for other users.

Hi,
Do you have a plugin or something to set custom user levels?
Is the user you are testing with a full Administrator user? Or maybe an Editor?
