I believe you are missing out something in the setting panel.
Please enable the below points-
A) WC catalog Enquiry general settings-
1) Catalog enable- checked
2) Product Enquiry Enable- Checked
3) Remove Price- Checked
4) Want a custom Button- Checked
5) Hide Cart Checkout Page- Checked
Enquiry Form Settings
6) Captcha Enable- Checked
7) Subject Field Enable- Checked
8) Phone Field Enable- Checked
9) Address Field Enable- Checked
10) Comment Field Enable- Checked
11) Redirect to other page- Checked
For further guidance, you can check the demo- Support and refer the setting panel to cross check if you have still missed out anything.

Nope – zero effect. All settings exactly as above (don’t want to hide price, but have tried with tried with and without “Remove Price” checked.

Hi Neil,
I have checked your theme products detail page and found the ” WooCommerce hook _single_product_summary” unavailable.
The catalog enquiry form is displayed using this hook and it’s a standard WooCommerce hook. Might be the theme developer did this and as a result, enquiry form is not getting displayed. So you can ask him to implement the hook back to your theme. In fact, you can cross check our Plugin with other standard WooCommerce theme like Storefront or Twenty sixteen etc to confirm the same.
Do let me know your views. I am awaiting your revert.

So, installed a brand new theme – it does take effect. However pop up window doesn’t work and when I use an inline form clicking “send enquiry” puts an overlay on top of the product page and the whole page is disabled for input… :/
