I was able to track down the issue. It was a plugin conflict. Specifically, an Affiliate Plugin that uses its own contact form, but the form is exclusive for the plugin. The plugin is Affiliates by Itthinx,

Ahh I see, I just tried and installed this plugin and also deactivated and re-activated Gutenberg Forms plugin without any issues.

I have the pro version of the plugin. Where you have to install their free plugin, buy their pro version of the plugin. Install the pro version of the plugin, deactivate the free one.
In addition, They have addons for their affiliate plugin, one is the woocommerce integration.
It’s definitely that plugin. If I deactivate that plugin your plugin works fine. maybe, it’s because I have the paid Pro version.
