Check your hosts error logs, if you can let me know what the error is, I might be able to direct you.
I got my host’s error log. It’s quite a lot of text, about 800KB. Do you want me to send it to you? I did a simple text search of it, and the words “error” or “internal” do not appear in it. Some of the entries do make reference to “add-from-server,” such as:

Those appear to be HTTP Access Logs, what’s needed is the PHP Error logs which are slightly different.
I don’t know where most hosts expose that data though.
