Invalid Header error won't allow plugin install through admin

I have an approved plugin: http://ww.wp.xz.cn/extend/plugins/serpd-vote-button/

For some reason when you install the plugin through WP admin it is adding another folder which is causing and installation error about header not being valid.

I don’t see where it is getting the extra folder for. It’s not in the subversion trunk… so why is it adding it???
