Issue with missing contact forms AMP

I have the Jetpack contact form on many of my AMP pages and it works great. Occasionally I will visit a page, and it will just say “add a form” in the place of the actual contact form. It seems like it might have something to do with page caching. My Redis full page cache is supposed to expire I believe after 8 hours so I wouldn’t think this should cause an issue. I just can’t figure out what is causing the occasional missing form. I worry a client will hit this blank form as well. Do you have any ideas?

It does appear to work well on my end at the moment, so it would indeed appear to be linked to caching on your site.
When you next see such a page on your site, I would recommend checking the page’s source code to find out more about caching on that page at the moment. Here is what I see right now, for example:
Once you have that information, it may be worth reaching out to the AMP team about this, and asking them if they’ve had other similar reports of conflicts between AMP and caching; they may have had some experiences with such issues, with Jetpack or other plugins adding elements to pages.
