You’re running into a bug that we’ve recently discovered. That said, it’s being fixed as I type this, and is planned for release next week. Here is the bug report on that one — https://givewp.featureos.app/p/givewp-4-9-0-resets-v2-forms-goal-min-max-amounts-and-donation-level-amounts
Rolling back the core GiveWP plugin to version 4.8.1 is the main workaround for now. You can use this link, then scroll to the bottom of the page to download the previous 4.8.1 version from there and install. The newer style Visual Form Builder (VFB) forms are not affected by this issue, only the older, option-based forms you are using. Since you mentioned that you still have to use the legacy version, you can either roll back to the previous version (4.8.1) for now, or wait for the patch next week.

I’ve downgraded the plugin to version 4.8.1, and the previous issue is now resolved. However, I’ve noticed another problem: when I select the form type “Multi-Step Form” (https://tinyurl.com/24rm6k8w), the Success/Error popup is missing(https://tinyurl.com/2btxlecx). With “Classic Form” everything works correctly.
Could you please check this issue as well?

A quick FYI to say our latest release, 4.10.0, solves the zero-amount values in the option-based forms for the Donation Amount and Levels.
Regarding your new question, I’m not able to replicate that. Here is a (silent) screencast starting from a campaign page with an option-based form in the multi-step format. Can you tell me if these the the same steps?
If so and its stuck on the spinner, can you check your browser console for errors, and let us know what they are with a screenshot please?

Thank you very much for the update! Yes, the issue with the amounts is fixed in the latest version.
Regarding my new question: the popup closes when a payment fails. In my case, the ARCA plugin makes some changes at this step, but I discovered that a style was breaking the popup content. Please see my screenshots.

Regarding the JavaScript errors, can you temporarily enable the Test Payment method in GiveWP > Settings > Payment Gateways on the Option-Based Form Editor tab, and briefly deactive the ARCA payment gateway plugin, and do a test donation and check your browser console for JS errors again?
