Issue with version 4.2

    Hi,

    The plugin is super useful and did not get any trouble until the new upgrade to version 4.2, I get the following warning:

    Undefined index: required in /wp-content/plugins/woocommerce-checkout-manager/includes/checkout.php on line 32

    Anyone experience the same issue?
