Issues using the latest version

    Hi!

    I never had no issues using Form Manger, but now the latest version threw some errors on my new WP installation.

    Issue details

    I just installed it on a new website on Hostgator and it threw following errors:

    1) Tried accessing the forms menu, it displayed an error stating there was a problem accessing database and it needs to reinstall – http://ge.tt/7oMlkVK2.
    2) After clicking the reinstall button, I saw another error- http://ge.tt/7oMlkVK2.

    Here are the details you may require:

    Website hosting provider : Hostgator
    WordPress version: 4.2.2
    FormBuilder version: 1.7.1
    PHP Version: 5.4

    I tried to install an older version (1.6.49) of the plugin, it has the same issue. Can you please check?
