This is possibly caused by the CSS removal of the grayscale button; do you have that enabled? It gets removed in Firefox even if enabled because it’s not supported by Firefox.

That works for me. I think that the grayscale button really only has value for testing, anyway.

First of all: great plugin for our non-profit organization!
I’ve tried turning the gray-scale button off and on.
On: it looks great in Firefox, but the gray-scale button shows up in ie.
Off: the buttons are on top off each other.
Turning all buttons off is not an option. Please advise.

For now, you can delete line 30 from the file /toolbar/css/a11y.css:
.a11y-toggle-fontsize { margin-top: -25px; }
This is a bug; I’ll get a fix out next week.
