This page (http://planetaempanada.com/pe/wpgforms/encuesta/) doesn’t make use of the Google Forms plugin. Am I missing something?

What happened is that clear because it does not work .

Recently install it again so you can see how it looks.

You need to downgrade your Google Form. There are two posts on my web site (this one and this one) which explain what you need to do.
