Surely your CSV is bad formed. The first two columns must be:
Username
Email

The main logo is bad, but all the rest is ok.
Could you tell me the other screenshot that is bad?
On the screenshot “Plugin page” you have the section “Columns position” – this is outdated too.
And to be on the safe side I would change the screenshot “Users imported” too, because there you have to old column order as well.
