Thanks for response but are you sure you’ve shared a correct form?
The one you shared is a very simple form, named “Registration” with just four fields and no paging. It doesn’t look anywhere close to what you are showing on the screenshot in your initial post.
Could you please double-check and share the correct form again so we could test it?
