Hi @sixaxis, thanks for the response.
I tried to upload a new media, with the exact same name and just 92KB.
I selected “just replace the file” and “Keep the date”.
I click on send and update the file, but the old media is still there. I have tried a many times..
My WordPress is 6.0.2 and my plugin is 4.0.0
@matcastelo, that is simply a cache issue. 🙂

It was a chache issue
Thats weird, I use Lite Speed to clear all the cache, but it was not working is this case.
