I have the same problem here. As soon as I turn on plugin, website stop working. jQuery monitor shows errors in the plugin code.
I’m still unable to add a tracking number. After pasting the tracking ID “Add New Tracking” button doesn’t work

Your store is still using version 1.27, you will have to upgrade to v1.28. It will resolve the issue.
