When you ran the upgrade did all your old forms were imported into the new plugin? Could you post a screenshot of the form you are referring to.

I reconnected my site — and this updated plugin — to my Campaign Monitor account. The good news is that the home page popup is back. URL:

I also had an e-mail newsletter subscribe form on the “thank you” page that contact form users are sent to.
Alas, the newsletter subscribe form has busted the page layout at:

It used to have a one-column band (in blue) above two columns, with the form in the right column.
Can you tell us what theme are you using? and what version or wordpress are you currently using we are taking a look at this.

I am using a customized child version of Divi. Just updated the Divi parent theme to the latest version. This was done a week before the release of the new Campaign Monitor plugin.
Also running the most recent WordPress release, 4.7.1. I updated it a week before the latest Campaign Monitor plugin came out.
As mentioned above, my home page subscription popup is back. It appears to be working in Firefox and Chrome.
