Layout stopped displaying correctly

I exported my Caldera Form from my site to a file on my computer and I then uploaded it back to the same site into a different Caldera Form on my site. After that my form lost its format of horizontal fields layout and now it displays as a vertical form.
Example of previous form before export of horizontal.
First Name Last Name Gender Date Of Birth

Now it displays like this,
First Name
Last Name
Gender
Date Of Birth

is there any way to get the horizontal layout fixed?
