Lead Generation plug-in for Gated Content

Is there any good plugins out there for lead generation forms with gated content? It needs to do the following in WordPress.

1) Build Form with custom fields
2) Send email to administrator(s)
3) Put information into a database that can be extracted to excel. Within wordpress is fine.
4) Once the user has submitted the form, it will redirect them to protected page with gated content, for downloading a whitepaper.

Preferably it should not allow an user to forward a url of the thank you page with the gated content. So they are forced to fill out the form.
