    Hi,

    Something in your CSS is causing this.
    Please give the following as Custom CSS, with a plugin, or inside theme CSS.

    #wpmchimpa .wpmchimpa-field select, #wpmchimpa input[type="text"]{
    height: 50px!important;
}
