Please update to 1.8.0. In plugin settings, under Apply modal tab you can add fileds and limit file types now.

We are getting an error when we attempt to use .docx, even though I have that box checked. The site returns “Error in submitting your application. Please try again later!”
PDF and .doc files work fine.
