Lines appearing when making booking

Hi,
This is a great plugin and just what I need. It is working well except when making the booking and on the ‘page’ where you select the time, enter your name, e-mail address etc. There seems to be extra lines on the form which shouldn’t be there.

Can anyone help or advise on what file to edit etc?
