Link back – sidebar

I’d like to have a section in my sidebar that allows user to add me to their websites/blogs, etc. I’d like it to have a few image options as well as a text box beneath so users can copy and paste the code (see this blog’s Link Back section (on the right towards the bottom) for reference: http://missmustardseed.com).

It can be done, but I’m at a loss as to how – widget? hard coding?
