link/lead tracking plugin for WordPress for unique partner links/codes

Hello,
I am not looking for a full eCommerce Affiliate program. I only really need a link/lead tracking plugin for WordPress 4.5.3.

Does your product meet my requirements below?

1) I have a site that does not have eCommerce. I have customer’s signing up for a service and we bill monthly after the service is provided.
2) I just want to be able to generate links (can last as long as I want) for each of my referral partners. These can be marketed on sites, email, twitter, etc. and should stay with the visitor (who clicked the link) throughout their time on the website and auto populate the referrer’s name or unique identification into a form.
3) I need to be able to generate unique links for each partner that I sign up. I am ok with manually being able to generate these links/codes, but could be open to an auto-referral partner link program in the future on my site.
4) What forms, auto-email do you work best with.
5) Can I store all information on my site or in my site database because some of the information that customers input in the form will be sensitive.
