I want to use this plugin for non-English speaking site. How could I change form labels?

You can translate any of the text used in the plugin by adding the languages to the “Development” section of https://translate.ww.wp.xz.cn/projects/wp-plugins/zendesk-request-form/dev
For example, if you wanted to translate text into German you would do that on this page.
