Hmm, if Login and 404 Lockouts are disabled, Defender should indeed not lock anyone out.
In the Firewall > IP Banning settings, are there any IP addresses related to your domain or server in the blocklist there?
If not, since you only need the Masked Login feature, please try a complete reset of the plugin in case some unwanted transients are maybe stuck in the database.
Go to Settings > Data & Settings and set the Uninstallation options there to Reset and Remove, and save.
Then click the Reset Settings button to clear all settings.
Now deactivate and delete Defender. Then install it again from Plugins > Add New.
That will sure you have a fresh install of the plugin.
Once re-installed, instead of clicking the Start Now button on the Defender start screen, click the Start from Scratch link so it doesn’t auto-configure things you don’t want. Configure only your Masked Login.
That should ensure you don’t get any more unwanted lockouts. 🙂
