Looking for a map plugin with specific feature

I’m looking for a map plugin that displays a map on a page, a visitor can then use a marker on the map to select their property – by postcode/zipcode which then sends the info to a form.

So the form is populated with the address chosen on the map

Does this exist? I’ve been through quite a few and finding it difficult to find one.
