Looking for plugin

I am trying to find a plugin that will allow me to post a ready made pdf that I assigned fields in with Adobe Acrobat Pro. I need the pdf to be able to be filled out then printed and emailed to an email address. I have tried a few of the plugins that I thought would do this but nothing is working like I need it to. I am not sure if I am not saving the file the right way in Acrobat pro for the names of the fields to carry over or if the plugin is just not picking them up.
