lost password e-mail problems

When a “lost password request” is issued, I get 2 e-mails.
The first contains only the following line:
Password Lost and Changed for user:
– – – – – – –
The second contains:
Login:
Password: 362207
Login at: http://action.counteveryvote.org/wp/wp-login.php
3 problems or concerns:
It looks like the first message is supposed to be the header for the second.
The login name is not inserted into the message.
The e-mail is sent from “[email protected]” – how do I configure the “from address”?
