You will need to use a form builder plugin such as Gravity Forms or Formidable Forms to accomplish this using a custom page.
There is also this article with instructions how to modify the default login page.

@binarywc Thank you for responding. I was hoping for a simple solution that didn’t require any additional plugins. I am not a developer and do not understand coding that much.
The forms builder plugins will be your best bet. You will just need to look at their documentation to figure out exactly how to do it. The only thing is the URL wont change. There are many plugins that will help you change the URL though.
Personally, I’ve never used a plugin for one purpose though. I typically use plugins for changing only one thing. If you do not want to use a forms builder plugin then it looks like LoginPress might be a good answer for your needs though.

I just created a dummy site and the lost password link work well sending users to this page https://giftinapotja.com/my-account/lost-password/ by default.
On my main site, it sends users to https://giftinapot.com/wp-login.php?action=lostpassword the default WordPress login page.
I am trying to cut down the number of plugins on my site and would like to stop using the Login press to mask the default WordPress login page.
