Mailchimp chackboxes not showing

Hi,

I have added a mailchimp form to my site and it has a opt-in checkbox at the bottom. However this checkbox does not show on the form on my site, but is present on the form in mailchimp.

I have tried deleting the checkbox re-adding it and also re-importing the list and still it does not appear.

Any help would be much appreciated.
