I noticed my e-mail opt-in has disappeared. When I looked into it my subscriber form is blank. I tried adding a new one, but this is also blank. I don’t know how to fix it.

I already uninstalled, deleted and reinstalled the MailPoet plug-in. Both the plug-in and my WordPress are up to date.

    How can I fix this?

Hey, manouk, is the form both blank in your WordPress admin and on the front-end of the site or is it only blank on the front-end?

Both. People cannot leave their e-mail and I only see a blank screen when I look at the form in my admin area. I also cannot make a new one, I keep seeing blank forms.
