Mailpress Form and Customising theme

I have just installed Mailpress and configured it but but there is only a widget for the form not Short code for that I can use in a Worpress page as the Subscription page.

Could anyone help on how to get the subscription form on a page.

Also, how do I customise the mail theme. I would like a the same template as site but I don’t know I to go about this.

Any help or documentation on this…?
