Good point. The plugin does not actually do the User Registration, or Login for that matter, leaving all of that up to WordPress.
WordPress does not offer manual approval of user self-registrations, but plugins do. I haven’t actually used any of the plugins, but I’ll see what other Private Site plugin users have said.

Although I have not tested the User Registration Approval process within it, I have worked with it before to make the Private Site plugin “aware” of Theme My Login’s special needs.
