You seem to be running an old version of Jetpack. Could you try to update to the most recent version, 7.1?
Once you’ve made the update, and if the problem remains, could you try to deactivate the Caldera Forms plugin for a few minutes, or remove the form form that page, and see if it helps?

I wonder if this is the same conflict we (Caldera Forms) have with Yoast? If so, and the 1.8.2 release that is coming up ASAP, doesn’t fix this, please open an issue in our GitHub.

I come again … no news for 6 day on the github … and my client web site still not showing the map on the contact page…

In this case, I recommend following up with Caldera’s support on this.
