Do you have JS enabled?
What are the settings you are using?
Did you try it with the Regenerate Thumbnail plugin disabled?

1. Yes. Javascript is enabled (Safari/OS X, latest versions). The bulk options are also not visible in Chrome 45/OS X. I don’t think this is a browser issue.
2. Very default. Please see this screenshot.
3. Yes. Doesn’t make a difference (except I know only have ‘Delete permanently’ as only bulk option.
I have enabled/added two MCM categories, as you can see in this particular screenshot.

Hai,
Thanks for the screenshots.
I noticed they look different from my own installation in that the arrow on the dropdown box is a single triangle for me as it is a double triangle in the screenshots.
Do you have any other plugins active?
Can you test with those plugins deactivated as well?
The usual test is to disable all plugins except for the WP MCM and check whether it is working. As long as it works, keep activating another plugin and testing again until it doesn’t work anymore.
As soon as it doesn’t work anymore, the last activated plugin would probably cause the error.
