Media Library Not Loading

Insert media hasn’t been working for me for around 2 months now. When I click on insert media, the media library does not load. The spinning icon just spins. When I insert a new upload, nothing happens after I click insert to post. I don’t have this problem with my other wordpress sites.

What I have done:
– Deactivated all plugins and cleared cache
– Switched to the twenty eleven theme and other themes to rule out theme-related reasons. Also emailed theme company about it and they said to post here on the wordpress forum since it is not theme specific..
– Changed file permissions 755 to 777
– Used different browsers (chrome, safari, and firefox)
