Members/Users Post Image Uploads Issue

When a member wishes to change their image on a post, they delete the image, upload new image, the image get’s uploaded and is added to the WP media library but it does not load on the post.

The initial uploaded image loads perfectly but if one attempts to upload a different one, it does not load at all. I have to do it myself via the WP media library..i do not see any image restriction settings anywhere..could you please help..
