Missing Form Filed Data upon submission

We’ll need to perform a further check in the form and fields configuration.
Please help us sharing the form export so that we could review the existing settings. Also include a list of the fields that are not getting submitted.

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.

You can share the export file via Google Drive, Dropbox or any cloud services in the next reply.

Hope you are doing fine!

I tested the form in my lab site and in the submission section all the fields are visible. Please see the image below:

However, in the email notification, there were some fields missing:

All you need to do is add the missing fields in the notification. Go to the Email Notifications tab > Add Email Notification > Email and in the Body field add the missing fields. Please see the following image as a reference.

In the image above, I am showing how to access the missing Detail and Medical History fields, just click on them to select them and add to the content.

Click on “Add” and next update the form. Test the notification and let us know if you get the expected results.
