Missing “Other” Amount Field

Hi. I love the plugin, and it’s exactly what I’m looking for. Unfortunately, the input field to enter an amount when choosing the other radio button is missing. The radio box is there, along with the “Other (p/m) label, but there’s no place for a person to enter the amount they wish do donate per month. If you choose the “other” option and hit submit, you get an error page on PayPal.

I’m not having any issues with any other input fields in my theme, and the input field when choosing the “Donate Once” tab is working correctly. I’ve also noticed that it’s missing on screenshots of the plugin at https://wp-ecommerce.net/wordpress-recurring-donation-plugin.
