This plugin should work find on mobile devices.
I have tested on my Android and iPad without any issues.
As with any website, mobile compatibility for an online form may need adjustments to the website styles (CSS) to work best.

I performed the tests with a Nokia lumia 500. It may be normal not to work, it’s very old.
Work with iphone 5.

With the Nokia, I would say it’s more to do with the age of the Internet browser than the age of the phone. I would think lots of things would not work as expected on it, like browsing form pages.
