Mobile form not working correctly

This form displays properly on all size platforms. On tablets and desktop, it is fully functional. On mobile (smartphone screens) the first 2 fields: the dropdown to choose a recipient, and the field to enter your name – do not work. The dropdown on mobile does not respond to the arrow activating the dropdown. The name field does not allow any entry at all. All the rest of the fields work fine on mobile. We have cleared all caches.
