• Thank you for this excellent plugin. My problem is that the default Submit form is too rigid to accommodate different types of respondent. In particular, I’m using the plugin for a 1-man service provider whose clients are all private home owners. So, I want to delete the Company Name field (or add “if any” to the label) and likewise the “Company Website field.

    I also want to add a new “Home Town” field but I would happily convert the “Heading” field to this label as I don’t want to mislead respondents into thinking that their choice of heading will also be mine.

    I hope that this locked thread will help me out – but I don’t know what I’m doing so it will be a ‘trial & error’ exercise for me, so presumably I should create a child php document for my theme?

    I’ll add that I think the form should include an opt-in checkbox which the respondent can check (or not) to allow his full name and contact details to be published (my alternative will be to publish only the respondent’s initials and his home town).

    I’ve got a similar question about the Widget fields but I’ll make a separate post.
