Do you use any caching plugin? If so, and is not possible to make exceptions in your caching plugin, then unfortunately CAPTCHA will not work correctly. If not, then:
1. Please try activating a standard WP theme for a while (2014, 2015) and check if the problem remains.
2. Try to deactivate all plugins temporarily except CAPTCHA to check if the problem remains. If it persists, then you need to activate your plugins one by one to find out what plugin causes the problem.
3. Provide a link to the page, on which the problem occurs.
